I was recently asked by the Dr. of our clinic to take over the billing completely.

I have very little experience, and would be doing it for all three clinics.  Can someone give me some advice.  How many people should be involved.  Does the main biller also handle the EOB"s?  Where would I get the best help on the internet?

3 clinics with little of bit of experience can be tough, But I'm sure you can handle it . I assume you are taking the roll of an Office manager, who will oversee the billing of the 3 clinics. Depnding on the volume of claims you have to deal with at each clinic will depend on how many people you need involved. The main biller doesn't have to handle the EOB"S however an individual who handles Payment Posting, Should also be the person calling insurances for denial or rejection of the claims. This particular job is very important and should be handled accordingly. When you look at billing. You need to have data entry (for posting the charges and posting EOBS) you need to have calle to rectify any issues with claims. You will need someone to interact with patients for any bills they may have and deal w/ paper claims and patient statements.

I'm an account representative for a medical billing company and we work w/ the front end staff to help w/ any particular part of the medical billing service. For example if you need help with posting payments. We do this for you and work with your staff. Communication with the front office is important to maintain a good revenue for a clinic/practice. Hello Kelly,

I am answering your post for advice on the new billing responsibility you are undertaking for three clinics. Here are a few initial questions for you to answer in order to determine the applicable manpower that will be required to efficiently manage this process while ensuring patient satisfaction and continuing cash flow:

 - How many patients are seen per month?

- What are the approximate charges per month by financial class? 

- How many individuals are currently scheduling patients and entering information?

Quick tip -- if you use batch eligibility processing to verify insurance and deductible information before patient appointments, you will save considerable time in eliminating unnecessary claim denials.  Also, to further efficientize your billing system, collect, in advance, deductibles and co-insurance payments from your patients if not already doing so. Both of these measures will dramatically reduce your accounts receivable and result in less man hours spent on back end follow ups.

Coming from over 15 years of billing and collections background, I have a good understanding of receivables management - - however, admittedly know little about "Medical Billing"....

 As I am currently looking to acquire a Medical Billing company, I have concerns where the industry is or may go with never-ending changes the insurance world.  Especially with the new Changing of the Guard - - Obama- - soon coming to office, I expect there will be many changes in many different areas.

 More specifically, my question is to all of the BLOGGERS.... what are the downfalls that may soon come to the Medical Billing industry that can dramatically change the climate of the business (for example, what SUB PRIME ultimately did to the Mortgage industry).  I am trying to further ensure this business is here to STAY and/or what risks there are with what may soon come down the pike interms of technology, Insurance carriers, government, etc...

Hi Everyone........I am looking at the opportunity to start a medical billing business.  I already have a home based buiness and have been in business for 14 years now with that business.  That business is with the Home Building Industry. So......I have been researching the opportunity with this company to start my own medical billing company.   

 I have performed a limited background check on this company listed above as Health Professionals Medical Billing Company out of New York.  But I don't know allot more than that.  I have spoken to a few of their established Billing Centers as they are called.  And it has been all positive.

 I would like to ask this of anyone who is willing to answer?  Do you know anything about this company you can share with me before I invest some up front money with these people. 

 And would you reccommend this industry as an industry to go into right now.  All the research I have done says yes.  What say you out there?  Please elaborate.

Bear in mind the following pointers before creating that critical purchasing decision for a new medical billing/practice management application: 

 First and foremost, consider how the technology will be utilized in the field, customization that is unique to your practice, and detail of automation that can be configured to fit your practice. 

For example, when do you bill your patients? A solid management practice system will bill the patient immediately after the insurance pays instead of waiting until month end. 

Another example is how often and when does communication occur during the patient encounter? A good management system will advise to the patient that their insurance has been billed in addition to an estimation of the patient's responsibility to pay. 

The last example of a good practice management system is when communication is sent to your patient after a rebill to their insurance has taken place. This serves as a courtesy and also prompts the patient to contact their insurance carrier regarding the delay in payment. Moreover, this will also serve as verification taht the insurance information still stands correct. 

In summary, technology has limits. It's the intelligent use of technology that can significantly improve accounts receivable and patient relations.

In case you haven't heard, the American Reinvestment and Recovery Act of 2009 (ARRA), more commonly known as the “Economic Stimulus Plan”, provides significant financial incentives to providers of care to Medicare and/or Medicaid patients for adoption of certified electronic health/medical record systems under the section of the Act known as HITECH.  It is widely assumed those incentives will be made available to providers already using EMR as well as those who adopt EMR systems in the future.  The largest incentive payments will be available to providers who adopt or have adopted EMR prior to 2011.

Select Collect: A novel and cost-effective approach for patient collection. The client designs their own collection process by selecting the activities that they would like to be performed on their accounts. Interestingly, these are most of the same techniques that may have been used on their accounts in the past by a conventional collection agency. However, in this program, the client retains 100% of the money collected and is only billed for the activities selected.

I was laid off by my previous employer and I am considering switching to a career in health care. Hi, am curious if anyone  knows of a way to bill for time spent reviewing medical records. For example, a patient comes to see me (I am a subspecialist) and brings over 100 pages of dense medical records to review. This happens often. I end up spending time after the consultation reviewing these old records because there was not enough time to do so during the intial visit. I really believe that so much time spent should be compensated - it totals many, many hours at the end of a week or month.  Rare that any other professional (lawyers as a prime example) would spend hours at the end of their work day reading through piles of paperwork without billing for ther time. Any ideas?? Thanks so much!

All I have to say is, just copy the medical systems most European countries have, France, Holland, Germany, Spain, Portugal, Britain the 4 Scandinavian Countries and the Swiss, even the ex est block countries have a better system that US has.
A civilized country must not tolerate this political BS and lies as it happens in the US. Every one must have medical coverage!
I have spent 34 years in Europe and never ever had to worry about my medical care and have to file bankruptcies due to medical bills.
If the Congress, The Senate and this administration will achieve the 100% Medical Coverage for all US Citizens, then I am sure that USA is an undeveloped nation, and all of us must stop saying that USA is the best country.
Same to all USA people if this issue will not be resolve by the end of this year. For the future, we may have consider voting for some new political parties with young folks and new ideas. All European nations have at list 5 and more political parties in their Parliaments.

I am confused with your situation because by reading what you said you are only taking over the medical billing portion of the process. What are they going to do about the coding portion of the process? Are you going to be taking car of that too?

If you are only going to take care of the billing portion then I suggest that you take a reputable online medical billing training course that lasts 3-8 weeks and preferably will allow you to sit, take and pass the Nationally Recognized Certified Medical Billing exam like the one I mention on my website.

If you have to also do the coding portion then I strongly suggest that you politely take a back seat and tell the practitioners that they must hire someone that is genuinely qualified to code. I know that may seem harsh right now but you will save yourself and them as a matter of fact from all the headaches you are not ready or trained to handle when problems arise (and they will).

Hello Sarah,

I am sorry to tell you this but you have the incorrect view of work from home medical billing. What you and so many others that are new to this industry think is that there are companies out there ready and willing to hire people to process their overflow of medical claims. This Is Not The Case!

People that do medical billing from home fall into two categories:

1)They were hired as a medical biller in an office environment that is set up to transmit and accept protected health information over a network. Once that biller becomes a trusted and efficient employee they could earn the right to work from home.

2)The other way to work from home doing medical billing is by starting your own medical billing business. The success or failure rests squarely on your shoulders since you are the President of the business. This is what is referred to in the industry as Home Based Medical Billing and that is why most have misconceptions about doing medical billing from home.

On another note, thank you for being so forthcoming about your home situation because I can tell you right now that even if you decide to start your own medical billing business in the beginning (1-3 years) or until you  sign your first client that actually puts you in profit, let me say that again IN PROFIT you will need to leave your home much more than you think (for example: cold calling, marketing, interviews, industry seminars, etc.).

Lastly, and this is me talking to you like a life long friend, if any school contacts me the red flags start flying! Why? Two reasons:

1)They are just big organizations with deep deep pockets that run solely on the fact they sign up so many people into their programs. The problem with that is that they are so big that they don't specialize in training you properly they train you to Just Be (an empty shell).

2)All the reputable medical billing training course owners and teachers that I am familiar with over the past ten years are taking their time to not only run their day to day business but they actually take additional time out their day to commit themselves to educate, mold and produce truly qualified medical billing professionals. I went to a local community college and the classes began on Oct 9 and ended on Dec 21 from 6-9:30 pm, Mon. and Wed.  So check your local colleges. I hope this helps.

1. Know your health plan contracts inside and out, especially the top 20 to 30 CPT codes that generate 90% of your revenue. Be proactive and track the reimbursement of these key codes to determine proposed pay cuts and voice your objections to the provider representative. You may be able to retain your current reimbursement. 

 

2. If rate decreases continue, increase your productivity. Analyze your patient flow to identify areas that can be streamlined, enhanced, or retooled. Also, some practices hire nurse practitioners or physician assistants who can do 80 percent of what a primary-care physician can do for less than half of what a physician earns. 

 

3. Bring previously outsourced ancillary services inhouse - only if it will turn a profit and is worth the time invested. 

 

4.  Consider seeing privately insured patients on a cash only basis.

It has taken me a very long time to realize that, with procedurally oriented practices such as mine, the billing company has no incentive to collect patient balances resulting from charges applied to deductibles. They only get their previously set percentage of around four to six percent collections, and this portion of the receivables is too labor intensive for them to go after. I will be bringing billing in house as soon as possible.

Without knowing the specifics of your situation, it sounds as if your experience with your billing company has been less than stellar, which means that your cash flow probably isn't as optimized as it could be, which could also mean that relationships are suffering. One immediate solution in the interim, until you move your billing in-house, is for your billing company and in-house staff to coordinate and immediately implement new patient protocols if your practice has more than a 1.5 % uncollectible rate on patient portions -- stick to the system and hold them accountable for results.

I work for a large company & run all the billing for my program. One of my clinicians has her own practice & asked me if I could do some of her billing. I would love to but I need help, do I need software? She does it herself right now & has no software. Will I benefit from buying? My most important question is this.. what do I charge? I'm lost, she said percentage or flat rate but I'm not sure what the percentage or flat rate should be. There isnt many claims & for right now I believe it will only be private insurance companies. Looking for specific benchmarks to hold outside billing company accountable. Currently, there is zero transparency. Physician group receives general retrospective reports from billing company and there is no way to verify what the billing company is doing.

Billing company currently reports DAR is 45 from the bill date. Any guidance from this website full of experts would be greatly appreciated.

You bring up a great topic. The obvious measurement is days in A/R. Depending on the specialty, days in A/R can vary. For example, our experience with practices heavy in personal injury or workman compensation cases is less than 34 days in A/R, when they take advantage of all of our resources.

Other metrics we deploy include:

- Require more specific data from your reports; drill down to details rather than general retrospective reports
- Timeliness of submitting charges to primary and secondary carriers
- Four patient statement cycles per month to assist in optimizing cash flow
- Follow-up on patient balances in line with protocols established by client
- Verification of insurance benefits BEFORE a patient schedules an appointment to eliminate rejections and inform patient of co-insurance and deductibles before each appointment. Thus, the patient is aware of their financial obligation and the practice can collect this portion at time of service.

A true partner billing company implements several checks and balances to ensure that transparent accountability is provided while optimizing their client's cash flow and enhancing patient relations.

I recently had lunch with someone who manages a medical office and she told me that because of the upcoming requirements for doctors offices and hospitals to electronically submit patient information to a national database, the need for independent medical billers will become nonexistent because the billing will be done automatically when the information to that database is submitted.

As part of your decision about an EMR purchase, now you have to determine if you want a client-server based or a web-based system.

A client-server based system is the one that we are the most accustomed to. You buy a server, purchase a license for the EMR software which you then install on that server.

I thought I understood the whole process of medical billing, but last time I went to the doctors for an annual visit, I was charged since I was diagnosed with a simple sight problem. Shouldn't a diagnosis be covered if its an annual visit? I hate to be billed when it should be covered.

Tip: Ensuring Payor Payment

Negotiate Rates. Insurance company pay rates usually renew annually. Even though renewal dates may fall at different times of the year for all of the companies, it is a good idea to compare all payors when one is up for renewal. To do this easily, prepare a spreadsheet containing all of the practice insurance company rates and monitor it closely.

However, it is easiest to negotiate with insurance companies during the renewal period. When negotiating use data from clinic expenses, prior increases and market conditions to prepare a case for increasing fees. Negotiate face-to-face if possible. If not, use email to follow up and document phone conversations.

Sometimes, just the awareness that the practice brings to payors goes a long way to obtaining the rate adjustments needed.

Under Obamacare, doctors can get a $44,000 reimbursement for switching to an EMR (electronic medical records) system.
The EMR software typically can run somewhere between $10,000 and $15,000. A doctors’ office can get $44,000 per doctor. If the office has four doctors, the office can get $176,000 best-case scenario. If the practice pays $10,000 or $15,000 for a software package, the practice still gets the $44,000 checks (for each doctor), paid over a three-year period. The longer it takes to get on the EMR system, the less of that $44,000 you get and there’s a schedule that tells us what that amount is. By 2014, you are required to be on an EMR system or you will not be able to get paid from the government. More importantly, the government is giving you the reimbursement of $44,000 per doctor to acquire this EMR system. You shouldn’t have any reason not to buy it. Next year, it will drop and then the year after that, it will drop again. The early bird gets the worm.
Still, doctors are hesitant. They realize EMR has a value and a purpose. It’s just that the transition and the learning curve slow them down. They have to get everybody trained on EMR. It may take some doctors four to five months to get through the whole process and get used to the new software. But like anything else, the only thing that is constant is change.
Sometimes the hesitation is justified and I have a solution for that. If you have been in practice 15-plus years, you’re going to have thousands of files of patient paper that you or your staff will have to convert. That’s going to be very costly and time-consuming. So what we suggest is that rather than trying to go back in to convert all those files into an EMR system, do this. Convert the files as each patient comes in.
